Services at Rae-Ann West Park
Our Behavioral Stabilization Program allows for shorter, in-patient hospital stays. An individual can transfer to Rae-Ann West Park after a qualifying hospital stay for an additional stabilization period in a secure, consistent environment conducive to behavioral and emotional wellness. Our goal is to return the individual to their highest level of functioning and discharge them home or to their other previous setting.
Specialized assessments for developing an individualized observational period with medication monitoring:
- Ongoing behavioral evaluations by staff and nurses
- Interdisciplinary team meetings with staff psychiatrist
- Specialized individual and group activities
- Discharge planning services
Additional Areas of Expertise
IV Therapy
Antibiotics, TPN, Peripheral IV, Multi-Lumen Central Catheters, Implantable Vascular Access Devices, VasCath, Peripherally Inserted Central Venous Catheter (PICC), and Hickman Catheters.
Medication Management
Multidisciplinary Medication Support.
Pain Management
Diagnosis and Treatment of Acute and Chronic Pain utilizing Non-Invasive Non-drug pain management (e.g. Exercise, Electrotherapy, Ultrasound, Infrared, and Massotherapies).
Hospice Services
Services provided to individuals in their end stage of illness. Support services are also provided by licensed Social Workers and Spiritual Care staff.
Skilled Nursing
Any service that must be performed by a Licensed or Registered Nurse (e.g. Wound Care and Diabetic Teaching). Along with any illness or injury needing Physical, Occupational and Speech Therapies.
Long-term Care
Need to be deficient in three activities of daily living to qualify for Long-term placement. Goal is to promote the highest level of functioning possible through individualized care planning.
Respite Care
A short term or transitional stay with all the necessary support care services for a potential resident. This stay may also provide a reprieve from support care services for a caregiver.
