Rae-Ann Westlake: Stroke Rehabilitation

Our Stroke Rehabilitation offers stroke victims intensive, personalized rehabilitation in the crucial time period following hospitalization. Housed in a separate wing of Rae-Ann Westlake, the Stroke Rehabilitation features tastefully decorated and spacious private rooms, bright and inviting common areas for visiting with loved ones, and a private dining room with a menu specially tailored to stroke patients.

Our focus is on rehabilitation and prevention of a recurrent stroke. With a neurologist on staff, residents do not have to leave the facility for follow-up care. Our experienced nurses and personal care aides are specially trained in the intricacies of post-stroke care. Physical therapists use special exercises, equipment and techniques to help residents achieve the highest functioning level obtainable. Occupational therapists work to teach activities of daily living. Speech therapists assess and treat problems with speech, language, swallowing, hearing, memory and attention deficit disorder.

Working as a team, our experts are devoted to crafting an individualized care plan to help each resident achieve maximum recovery and independence.

IV Therapy
Antibiotics, TPN, Peripheral IV, Multi-Lumen Central Catheters, Implantable Vascular Access Devices, Peripherally Inserted Central Venous Catheter (PICC), and Hickman Catheters.

Medication Management
Multidisciplinary Medication Support.

Pain Management
Diagnosis and Treatment of Acute and Chronic Pain utilizing Non-Invasive Non-drug pain management (e.g. Exercise, Electrotherapy, and Massotherapies).

Hospice Services
Services provided to individuals in their end stage of illness. Support services are also provided by licensed Social Workers and Spiritual Care staff.

Skilled Nursing
Any service that must be performed by a Licensed or Registered Nurse (e.g. Wound Care and Diabetic Teaching). Along with any illness or injury needing Physical, Occupational and Speech Therapies.

Long-term Care
Goal is to promote the highest level of functioning possible through individualized care planning.

Respite Care
A short term or transitional stay with all the necessary support care services for a potential resident. This stay may also provide a reprieve from support care services for a caregiver.

Psychological Services
Psychiatrist and Psychologist on staff.

Dialysis
Peritoneal. Care and monitoring for Hemodialysis.

Dementia
Specialized care in handling all stages of Dementia.
