Rae-Ann has offered the finest in skilled nursing services for over 30 years. All of our staff are Rae-Ann employees, not agency personnel, and many have been with us for decades. We maintain a high staff-to resident ratio and design every aspect of our program around the needs of our residents. A customized plan of care is prepared for each individual. Whether the person is a long-term resident or here for a short stay, we work to meet individual goals by providing the best therapies available. Our team continually re-evaluates each resident’s status and prepares for the next level of care as progress is made.

Excellent State Inspection History

We pride ourselves on our outstanding Ohio Department of Health annual inspection survey history. The State of Ohio oversees the licensing of nursing homes and conducts unannounced onsite inspections approximately once each year of facilities that serve Medicare and Medicaid beneficiaries. Inspectors monitor over 150 regulatory standards related to quality of care, staff/resident interaction, and facility conditions.
